Summary
The Department of Medicine Section of Pulmonary, Critical Care, and Sleep Medicine is seeking a part-time pharmacist to work with the Baylor College of Medicine (BCM) Adult Cystic Fibrosis (CF) Center. The BCM Adult CF Center provides care for nearly 400 patients and is located at Baylor McNair Campus at the heart of the Texas Medical Center. Patients with Cystic Fibrosis have complex needs and utilize a variety of pharmacotherapies in management of their disease, some of which require monitoring for dangerous side effects and significant medication interactions. The CF program pharmacist role aims to address deficits in patient medication access, increase review of pertinent drug interactions, and improve patient adherence to therapy.
This position will work around this schedule based on clinic hours:
Monday AM 4 hours
Tuesday AM 4 hours (Huddle starts at 8 am and usually take 2 hours)
Wednesday PM 4 hours
Thursday AM or Tuesday PM 4 hours
Job Duties
The CF pharmacist role includes the following duties:
- Performs thorough patient medication review.
- Creates and maintains standardized process to prevent, detect, and document relevant medication effects, including adverse effects, allergic reactions, medication interactions, abnormal monitoring parameters, etc.
- Interprets physician's orders for medications and ensures therapy is appropriate for patient's diagnosis,weight, and age.
- Identifies barriers to medication access and connects patients with appropriate medication assistance programs, where needed.
- Provides medication education to patients and providers.
- Maintains outpatient IV antibiotic monitoring program, relevant to patients with Cystic Fibrosis
- May participate in pharmaceutical continuing education, in accordance with departmental performance standards.
- May conduct/participate in research activities to include publishing findings in professional journals.
- Attends weekly patient huddles.
- Attends and participates in two half day CF clinics
- Attends and involvement in North American CF Conference (NACFC)
- Collaborates with physicians and team members as part of a cohesive team care model
- Participates in CF related quality improvement and research projects
- Performs related duties as required.
Minimum Qualifications
- Bachelor’s degree in Pharmacy or Pharm.D. degree from an accredited school of Pharmacy.
- No experience required.
- Current licensure from the Texas State Board of Pharmacy.
Preferred Qualifications
- One year experience as clinical pharmacist.
- CF pharmacy experience.
