Summary
The Department of Medicine Section of Pulmonary, Critical Care, and Sleep Medicine is seeking a full-time case worker or case manager with at least one year of experience to work with the Baylor College of Medicine (BCM) Adult Cystic Fibrosis Center. The BCM Adult CF Center provides care for nearly 400 patients and is located at Baylor McNair Campus at the heart of the Texas Medical Center. Cystic fibrosis patients struggle with mental health and psychosocial difficulties as well as finding and maintaining consistent and appropriate insurance resources, all which impact overall health greatly. We are looking for an individual who works well in a multidisciplinary team to provide the care and assistance these patients and families in need.

Job Duties
- Prepares and maintains accurate, HIPAA compliant patient records, charts and documents .
- Conducts mental health screening in alignment with CF Foundation mental health care guidelines.
- Coordinates mental health interventions and referrals:
- Performs crisis management for those patients scoring in the severe range on screening.
- Develops network of community based mental health providers.
- Assesses insurance coverage status and assist patients in obtaining and maintaining appropriate health care coverage.
- Attends three half days of clinic per week.
- Documents patient assessments.
- Performs follow up on active patient issues.
- Attends weekly operations meeting and patient care huddles:
- Collaborates with physicians and team members as part of a cohesive team care model.
- Participates in CF related quality improvement and research projects.
- Performs other job related duties as assigned.
Minimum Qualifications
- Bachelor's degree in Social Work, Counseling or in a related field.
- One year of relevant experience.
- Licensed Baccalaureate Social Worker (LBSW) or Licensed Master's Social Worker (LMSW) by the Texas State Board of Social Worker Examiners (TSBSWE).
Preferred Qualifications
- Master's degree in Social Work from a school approved by the Council on Social Work Education.
