Summary
Nurse Family Partnership (NFP) is an evidence-based community health program that partners Registered Nurses with first-time, low-income Mothers to improve outcomes for both mother and child. The program begins during pregnancy and continues through the child's second birthday. This position plays a critical leadership role in ensuring the successful implementation of the Nurse-Family Partnership (NFP) model. It requires a unique combination of clinical expertise, program management skills, and experience in mentoring and supervising nursing and administrative staff.
The Nurse-Family Partnership Program at the Baylor Teen Health Clinic is seeking a Nurse Supervisor to lead and support a team of Nurse Home Visitors and Administrative Staff. This role involves overseeing recruitment, training, supervision, and performance management while ensuring fidelity to the Nurse-Family Partnership (NFP) model. The Nurse Supervisor will also perform occasional home visits, coordinate quality improvement strategies, and collaborate with community advisory boards. Ideal candidates will bring strong clinical leadership and a passion for serving high-risk, low-income populations.
The ideal candidate will have experience with high-risk populations, and also demonstrate strong communication, coaching, and problem-solving abilities. Prior experience in maternal-child health, pediatrics, or mental health is especially valuable due to the sensitive and complex nature of the client population served. This position also demands a high degree of accountability for maintaining data accuracy, quality improvement practices, and active community engagement. A background in public health or a Master’s degree in a health-related field would further support program goals and long-term impact.
Job Duties
- Supervises a team of 5 nurse home visitors and 1 administrative data support staff, providing performance feedback, coaching, and professional development plans to enhance individual and team effectiveness.
- Oversees the recruitment, interviewing, and hiring process for nurse visitors and administrative staff, ensuring alignment with the Nurse-Family Partnership (NFP) philosophy and service delivery model.
- Conducts regular evaluations and one-on-one supervision sessions to monitor staff adherence to the NFP model and address any clinical or operational concerns.
- Performs select home visits to support high-risk, low-income clients and model effective service delivery in accordance with NFP Visit-to-Visit Guidelines.
- Ensures accurate, timely data entry into the web-based NFP data system and reviews reports to guide quality improvement and compliance with national standards.
- Coordinates and participates in community outreach, coalition meetings, and advisory board activities to promote program visibility and client referrals.
- Collaborates with leadership to implement quality improvement initiatives and report program outcomes to stakeholders.
- Manages or contributes to grant-related activities, program budgets, and data audits as needed.
- Adheres to nursing processes and the Nurse Family Partnership (NFP) model of home visitation.
- Assists with the recruitment, interviewing, selection, and hiring of all nurse home visitors and administrative data support staff.
- Provides supervision and evaluation of nurse home visitors and administrative data support staff, including developmental feedback and performance issue resolution.
- Identifies and assesses staff development needs, providing coaching, mentoring, or formal training to improve knowledge and skills.
- Performs home visiting in accordance with the NFP model and Visit-to-Visit Guidelines.
- Oversees timely and accurate data input and report generation through NFP systems.
- Implements quality improvement strategies and participates in community advisory board meetings.
- Develops and maintains appropriate materials for community and client education.
- Performs other duties as assigned.
Minimum Qualifications
- Associate’s Degree or Bachelor’s degree in Nursing.
- Three years of relevant experience as a Registered Nurse (RN).
- Current license as a Registered Nurse (RN) by the State of Texas Board of Nursing.
- Basic Life Support (BLS).
- Certification in area of specialty for non-BSN incumbents.
Preferred Qualifications
- Master’s Degree in Public Health or a related health field.
- Program management/coordination, one-on-one supervision, and experience with maternal-child, pediatrics, or mental health populations.
