Title: Senior Medical Social Worker
Summary
The Department of Neurology at Baylor College of Medicine is seeking a motivated, qualified social worker to support the General and Neuromuscular section including Muscular Dystrophy Association Care Center and Cure SMA Care Center. Will be available to provide a social worker evaluation and participate in creation of treatment plans for any Muscular Dystrophy Association patient at the time of the Muscular Dystrophy Association Clinic.
The ideal candidate will have the ability to display a professional, positive attitude with strong communication skills. Must be able to work independently and collaboratively. Lastly will be detail-oriented, self-motivated, efficient, and strong organizational skills.
Job Duties
- Serves as a liaison between medical and nursing staff, patients, caretakers, and appropriate outside agencies.
- Maintains the neuromuscular patient database, completes patient consent documentation, and uploads required information.
- Arranges transportation for clients to physicians’ appointments and follows-up with patient who miss scheduled appointments.
- Completes psychosocial assessment, primarily for therapeutic support and to remove barriers to care (e.g., barriers to following the medical plan, medical plan adherence etc.)
- Maintains big picture of clinical services to help facilitate communication between providers.
- Assists with scheduling family meetings for medical decision making.
- May assist family with medical decision-making including end of life discussions, palliative care meetings, and advance directives.
- Supports the insurance verification process including physician letters for insurance advocacy, Letters of Medical Necessity, immigration, charity applications.
- Collaborates with patients to complete medical forms (e.g., FMLA, disability, employment, etc.)
- Continues to learn and enhance current knowledge of local resources and relevant medical research to provide best possible recommendation to patients.
Minimum Qualifications
- Master's degree in Social Work from a school approved by the Council on Social Work Education.
- One year of relevant experience.
- Licensure as a Master Social Worker (LMSW) by The Texas State Board of Social Worker Examiners (TSBSWE).
