Summary

The Registered Nurse II will be working in the Division of Pediatrics-Public Health on a new, 4-year prevention grant received from the state of Texas (upSTART Program). The Registered Nurse II will be implementing the evidence-based Family Connects home visiting protocol through conducting home visits to families and newborns in the Houston region. The Registered Nurse II will assess families in 12 domains of physical and psychosocial well-being, including health and physical assessment of postpartum mothers and newborn babies in their home, assessing risk factors for the family, establishing a family support plan, assisting families in connecting to community agencies and appointments, and responding to client needs through flexible scheduling of home visits. 

Job Duties

  • Implements the evidence based Family Connects home visiting protocol, which includes a minimum of 6 nurse home visits per week.
  • Uses the nursing process in nursing assessments, planning, implementation/intervention, education, evaluation and consultation.
  • Follows the Family Connects scheduling protocol, including productivity requirements as outlined by supervisor.
  • Coordinates with multidisciplinary team to recruit families for the program, schedules home visiting sessions, and contacts families for follow up as needed.
  • Provides health and physical assessment of mother and baby in their home to determine well-being; assesses family needs in 12 domains of physical and psychosocial well-being, including depression, substance abuse, and domestic violencel exercises clinical skills in appraising situations and making decisions.
  • Keeps up-to-date client records and documents assessments, visits, referrals, and follow-up in medical record, program, and state databases.
  • Maintains confidentiality per HIPAA guidelines in regards to patient information.
  • Establishes a trusting relationship with families during home visits and engages families to establish support plans with goals, objectives and activities that address family strengths and needs.
  • Applies working knowledge of parent-child interaction, child-maternal health, child development, and child maltreatment prevention to help families improve parenting skills, increase parenting supports, reduce stress, and address needs of parents; assists in creating a health plan for entire family.
  • Works with a multidisciplinary team by attending weekly team meetings, clinical supervision, staff meetings, and other meetings as required. 
  • Adheres to the scope of practice as defined by the Texas Nurse Practice Act.
  • Completes all BCM institutional training requirements and completes all annual training requirements as required.
  • Becomes fully trained in Family Connects home visiting protocol as stated in the current program implementation manual; this may initially include travel to Durham, NC, for observations, training and “certification."
  • Attends in-services or continuing nursing education programs which benefits nurse’s knowledge and client care. 
  • Maintains continuing nursing education units to comply with standards set by the Texas Board of Nursing, as evidenced by and proof of attendance.
  • Attends Division meetings.
  • Represents the Division, and the upSTART and Family Connects program at community events and community.

Minimum Qualifications

  • Associate's Degree or Bachelor's Degree in Nursing.
  • Two years of relevant experience as a licensed Registered Nurse (RN). Degrees in lieu of experience will not be accepted.
  • Current license as a Registered Nurse (RN) by the State of Texas Board of Nursing.
  • Basic Life Support (BLS) Certification.

Preferred Qualifications

  • At least two years of relevant experience in clinic, hospital and/or public health nursing as a Registered Nurse (RN), preferably with postpartum women and/or families with newborns.
  • Bilingual (Spanish/English).
  • Valid Texas driver’s license with an acceptable driving record.
  • Ability to travel to more than one work location.
  • Proficient and comfortable using laptops as part of in-home assessment and intervention.
  • Clinical experience related to domestic violence, substance abuse, maternal depression, maternal/child health, and/or trauma informed and family centered care.
  • International Board Certified Lactation Consultant (IBCLC).
  • Ability to work with a culturally diverse population.
  • Understanding of equity, root causes of health disparities and social determinants of health.
  • Understanding and commitment to a universal home visiting program as a public health intervention for newborns and their mothers.
  • Experience working with community based organizations.
  • Ability to work with clinical and non-clinical staff.
