Under the direction of the Medical Director, the employee is a registered professional nurse level I who provides in-office support to a primary care practice as part of a Team Based Care model for the delivery of care with the patient as the central focus.  This position will triage walk-in clients and provide telephone advice to patients with medical questions or concerns in conjunction with the HHH clinical providers. The Registered Nurse I will serve as a patient educator, may assist with monitoring acutely ill patients during the patient visit, and assist with procedures that are within the scope of the professional nursing practice. This position will also support the on-going management of patients with stable chronic diseases. 


Busy clinical environment caring for the adult homeless population with an age range of 18-80's with occasional pediatrics.

Job Duties

•Serves as a resource for teams of MA and Provider as part of an extended team that consists of other support staff to triage walk in patients. Manages the intake of patients with acute medical and/or psychiatric complaints alongside the PCP, APN/PA.
•Provides protocol based RN visits as supervised by the Medical Officer. 
•Provides patient education in areas such as self-care; e.g. taking medications appropriately, use of nebulizers and inhalers, diabetic education, wound care, and self-injections. 
•Oversees the intake of patient telephone calls, follows established telephone and departmental guidelines, provides advice regarding treatment of certain minor acute problems. Refers questions to the PCP or APN/PA as needed. Provides on-going assistance to support staff, MA's and supervisors in areas such as proper and thorough message taking and emergency telephone protocols. 
•Participates in patient follow-up activities ie. diagnostic test results, arranging for prescription refills, expediting referrals, performing prior authorizations. Prepares information for prescription renewals at the request of the PCP/APC.
•Participates with on-going management of patients with stable chronic diseases, as designated by and in consultation with the PCP/APC. Maintains chronic disease rosters for the practice. 
•Performs procedures as determined by the clinical needs of the department, including but not limited to: IM and SC injections, nebulizer and inhaler treatments, EKG's, administering and reading PPD's, suture removals, ear lavage, wound care/dressing changes, and other procedures that are within the scope of professional nursing practice.
•Serves as resource to MA's, SW, CHW and patient navigation to services within the practice.
•Participates in patient care quality improvement projects as requested.
•Maintains BLS certification.
•May review published clinical reminder lists and act according to established protocols, e.g. Flu vaccine reminders, overdue mammograms, etc.
•Performs all job functions in compliance with applicable federal, state, local and company policies and procedures.

Minimum Qualifications

  • Associate's Degree or Bachelor's Degree in Nursing.
  • No experience required.
  • Current license as a Registered Nurse (RN) by the State of Texas Board of Nursing.
  • Basic Life Support (BLS) Certification.

Preferred Qualifications

  • BSN preferred from a state approved school of nursing.
