Summary
The Nurse Care Manager serves as a member of the Care Navigation team serving high-touch Baylor constituents, special patient populations, and corporate clients, facilitating an optimal patient experience. Coordinates full facilitation and case management of incoming patients and bridges communications efficiently between patients, physicians, multiple providers, clinical staff, and external resources.
The ideal candidat must have excellent interpersonal communication skills and have the ability to teach, analyze and problem solve with relative ease. Should also have experience with special patient populations (executives, donors, international patients, and other constituents).
This position is also located at our McNair and Woodlands Campus. Additionally, will work remotely one day a week.
Job Duties
- Provides individualized assistance to patients, families, and caregivers to help overcome health care system constraints and facilitate timely access to quality medical care from screening and management of care.
- Maintains the healthcare management of defined number of patients, identifying gaps in screening and preventive care, coordinating follow-up visits, and regular communication.
- Works closely with physicians in all specialty areas to coordinate patient's care plan communication; works with multidisciplinary team to maintain and implement up-to-date coordinated care plan.
- Obtains medical records, imaging/pathology and insurance information to create single source medical record management for treatment team/patient access.
- Actively involves the patient/family in the plan of care and conducts patient/family education as appropriate.
- Develops relationships with physicians and serves as a liaison between specialty departments and patient's referring physician.
- Provides telephone triage and ensures patients receive the right level of care and best-suited provider in a time frame that is clinically appropriate.

Participates in on-call support in once monthly rotations. - Partners with the Business Development staff to coordinate relationships with external/referring physicians for the program, as appropriate.
- Actively participates in a wide range of performance improvement and shared governance activities by identifying, recommending and leading improvements in patient care processes at service line level.
- Ensures appropriate data is collected and submitted to appropriate databases for service lines supported.
- Independently manages time effectively and prioritize daily tasks, assisting coworkers as needed.
- Performs other duties as assigned.
Minimum Qualifications
- Bachelor's degree in Nursing.
- Four years of relevant experience.
- Current license as a Registered Nurse (RN) by the State of Texas Board of Nursing.
- Basic Life Support (BLS) certification from the American Heart Association.
Preferred Qualifications
- Eligibility or Certification in Case Management by the Texas Department of State Health Services (DSHS).
