Summary
This position provides advanced care to patients in a clinical (outpatient) and hospital (inpatient) settings in an area of specialization under the direction of the physicians of the Heart Failure, LVAD, and Heart Transplant Team. This position is only working at Baylor St. Luke's Medical Center in collaboration with all members of cardiology, including physicians, advanced level providers, nursing team, coordinators, and patients, in a manner that is effective, professional, and friendly.
Job Duties
This position will be involved in direct patient care, independently and along physicians in the inpatient care areas (ICU, Acute Care, telemetry) and Outpatient Clinic and is responsible for:
- Obtain detailed history and perform physical examinations
- Perform, order, and interpret diagnostic procedures in accordance with relevant health concerns of the patient.
- Prescribes appropriate pharmacological and non-pharmacological treatments.
- Maintain records and write orders and progress in patient charts as to status, treatments, and procedures
- Monitors and ensures accuracy of recording on patient medical records.
- Provide follow-up health maintenance care to patients in accordance with protocols established with the supervising physician.
- Communicate with physicians, patients, and the family members of patients concerning patient care.
- Collaborates with providers, and practice staff in identifying appropriate patients for care management.
- Perform other job-related duties as assigned.
Minimum Qualifications
- Must have clinical experience in specialized care of an adult population and have completed an advanced level educational program (Physician Assistant - Certified or Acute Care for NPs)
- Master's Degree or Bachelor's Degree with equivalent experience, as required per licensure.
- Current license to practice in the State of Texas
- Appropriate board certification
