Title: Medical Social Worker
Summary
The Section of Transition Medicine is seeking a Social Worker to coordinate and provide social services for patients and families. This position provides day to day case management to help patients and families, conduct interviews and assess patients and families to obtain social history relevant to medical problems and health condition.
The ideal candidate will be skilled with the Microsoft Office Suite Programs (Excel, Outlook, PowerPoint and Word), will have experience interacting with individuals with intellectual or developmental disabilities and is bilingual in English/Spanish.
Job Duties
- Performs appropriate assessments for patients and families in need of services.
- Assists patients in achieving positive psychological or social adjustments.
- Arranges transportation for patients to physician appointments.
- Serves as a liaison and effectively communicate between medical and nursing staff, patients, family members, guardians, and appropriate outside agencies.
- Maintains knowledge of insurance and benefits eligibility to assists patients and family with obtaining community resources, and accessing state, local, and federal programs.
- Maintains timely, accurate, and appropriate documentation on patient office/video visits or phone calls in EPIC (electronic medical records system).
- Participates in planning and implementing quality improvement activities.
- May supervise social work students.
Minimum Qualifications
- Bachelor's degree in Social Work, Counseling or in a related field.
- One year of relevant experience.
- Licensed Baccalaureate Social Worker (LBSW) or Licensed Master's Social Worker (LMSW) by the Texas State Board of Social Worker Examiners (TSBSWE).
Preferred Qualifications
- Master's degree in Social Work from a school approved by the Council on Social Work Education.
- Professional or personal experience interacting with individuals with intellectual or developmental disabilities.
- Bilingual in English/Spanish.
