Summary
The section of General Internal Medicine is seeking a competent and reliable Medical Social Worker to be a part of the GIM group to support our clinicians providing services to patients. Under direct supervision, the Medical Social Worker interviews and assesses client and relatives to obtain history relevant to medical problems and history.
This position is located at our McNair Campus.
Job Duties
- Assesses GIM adult outpatients during initial contact, interviews patients and relatives to obtain social history relevant to medical problems and history, and documents progress after client's visits.
- Answers patient calls and assesses need for services and arranges transportation to physicians' appointments.
- Refers patients to appropriate agencies and resources that can assist with non-medical issues and provide social support.
- Advocates for patient and family empowerment and independence to make autonomous health care decisions and access needed services within the health care system.
- Serves as a liaison between medical and nursing staff, clients, relatives and appropriate outside agencies.
- Provides adult patients and families at GIM McNair Clinic with information about their medical conditions and available social services to help them navigate those services and the healthcare system.
- Provides patients with counseling, guidance and assistance with interpersonal, emotional and financial problems.
- Refers patients to government agencies and nonprofit organizations that can assist with homelessness, food insecurity, unemployment or lack of transportation. Examples include, but are not limited to: Houston Food Bank, Houston Area Urban League, Supplemental Security Income (SSI), etc.
- Performs other job related duties as assigned.
Minimum Qualifications
- Bachelor's degree in Social Work, Counseling or in a related field.
- One year of relevant experience.
- Licensed Baccalaureate Social Worker (LBSW) or Licensed Master's Social Worker (LMSW) by the Texas State Board of Social Worker Examiners (TSBSWE).
Preferred Qualifications
- Master's degree in Social Work from a school approved by the Council on Social Work Education.
