Summary

The Community Health Worker (CHW) will be working in the Public Health Pediatrics Department on prevention grants funded by the State of Texas and Harris County. The CHW will provide support to upSTART staff from all three programs provided through the grants: upWORDS, a group based early language development program; upLIFT, a home visitation program provided to pregnant/postpartum women with elevated symptoms of depression/anxiety; and Family Connects, a brief home visitation program for families with a newborn. The CHW will work with staff and participants to connect families to needed services within the community.

Job Duties

  • Supports staff and participants from upSTART (estimated to serve 180 per year).
  • Assists front-line staff with identification and connection to resources for participating families.
  • Develops relationships with community organizations and facilitates a warm hand off to families.
  • Identifies and connects with community organizations for infant and maternal care supplies.
  • Develops and maintains a resource list for common goods/services needed in the grant service areas.
  • Promotes upSTART programs at community events.
  • Collects program data and enters the data into the program and state databases.
  • Coordinates with the project staff to schedule visits, send reminders, and contact participants for follow-up as needed.
  • Recruits and enrolls participants at hospital for Family Connects, an upSTART program.
  • Helps participants connect with resources in their area and assists with applications for eligible programs.
  • Attends community advisory board meetings, as necessary, to represent the Department, share resources, and gather resources. 
  • Documents all activities, service plans, and results.
  • Maintains confidentiality per HIPAA guidelines in regards to patient information.
  • Attends program staff meetings.
  • Attends Department meetings.
  • Represents the Department and the upSTART at community events.

Minimum Qualifications

  • High School diploma or GED.
  • Two years of relevant experience in health/social services.

Preferred Qualifications

  • Bachelor's degree in Social Work.
  • Community Health Worker Certification.
  • Bilingual (Spanish/English).
