Summary

The Community Health Worker (CHW) will be working in the Division of Pediatrics-Public Health on a 2-year prevention grant from the state of Texas. The CHW will be providing support to the upREACH+ program, a brief home visitation program for pregnant women and families with a newborn. upREACH+ helps families prepare to bring home their newborn, support maternal health and well-being during pregnancy and the early postpartum period, and help ensure families have the information, resources, and connections to community resources to promote family wellbeing. In this program, a nurse and a CHW meet with the family together in the family’s home and/or virtually to provide support and resources. Specifically, the CHW will recruit participants from a hospital and assist them with care coordination and accessing community resources.

Job Duties

  • Assists with recruitment and enrollment of participants in a hospital or clinical setting.
  • Assists front-line staff with identification and connection to resources for participating families.
  • Develops relationships with community organizations and facilitates a warm hand off to families.
  • Identifies and connects with community organizations for infant and maternal care supplies .
  • Develops and maintains a resource list for common goods/services needed in the grant service areas.
  • Promotes upSTART programs at community events.
  • Collects program data and enters the data into the program and state databases.
  • Coordinates with the project staff to schedule visits, send reminders, and contact participants for follow-up as needed.
  • Helps participants connect with resources in their area and assists with applications for eligible programs.
  • Documents all activities, service plans, and results.
  • Maintains confidentiality per HIPAA guidelines in regards to patient information.
  • Attends program staff meetings.

Minimum Qualifications

  • High School diploma or GED.
  • Two years of relevant experience in health/social services.

  • Bilingual (English/Spanish)

Preferred Qualifications

  • Bachelor's degree in Social Work.
  • Community Health Worker certification
