Title: Case Worker
Summary
This position will play a vital role within a new program that is aimed to expand the mental health workforce by training specialty providers in the field of Addictions and Substance Use Disorders. The Case Worker will act as a Peer Support Specialist, working collaborating with a team of healthcare professionals to provide peer support and contribute to team-based clinical treatment for individuals in underserved areas of Houston for health care needs, and other critical resources. The ideal candidate would be a certified Peer Recovery Support Specialist (RSPS, PRSS, or RSPS-I), and or/ Mental Health Specialist (MHPS), and be proficient in English and Spanish.
This position is located at El Centro de Corazon, Magnolia Health Center, 7037 Capitol St., Ste. N100, Houston, Texas 77011.
Job Duties
- Coordinates with clinic staff to provide support services to clients with mental health, and/or substance abuse problems. Works as part of a team that includes Addiction Psychiatry fellow-level trainees, addictions therapists, and other professionals offering team-based clinical treatment.
- Performs assessments of client needs, and with client, develops a negotiated care plan.
- Assists patients in achieving their personal recovery goals by promoting self-determination, personal responsibility, and empowerment using group and individual intervention.
- Serves as client advocate to community service providers to facilitate client access to needed services. Assist in reducing stigma and promoting awareness.
- Provides education and information to individuals, families, local schools, and medical providers.
- Provides individual recovery coaching when indicated by the treatment team.
- Maintains accurate documentation and and interactions with clients while adhering to confidentiality and privacy regulations.
- Participates in training and continuing education to stay current with best practices in peer support sand recovery.
Minimum Qualifications
- Bachelor's degree in Social Work, Psychology, Human Services or a related field.
- Three years of relevant experience.
Preferred Qualifications
- Master's degree in Social Work, Psychology, Human Services or a related field.
- Certified Peer Recovery Support Specialist (RSPS,PRSS, or RSPS-I), and/or Mental Health Peer Specialist (MHPS).
- Fluent in English/Spanish.
