Summary
The Case Worker will provide help in accessing available resources, development of care plans, information referral, and advocacy for clients. Works with clinicians and other staff to provide clients access to a continuum of care addressing health care needs and other critical resources, such as therapy and psychiatry services, basic needs, entitlement benefits, and other long term care needs.
Job Duties
- Works closely with clinic staff as well as city and county agencies to provide support services to clients with mental health, physical health, and/or substance abuse problems.
- Performs assessments of client needs, and with treatment team and client, develops a negotiated care plan.
- Evaluates and/or revises care plan as indicated with appropriate documentation in progress notes and communicates to necessary team members.
- Builds and maintains a therapeutic relationship with clients through engagement and negotiated goal setting.
- Serves as client advocate to community service providers to facilitate client access to needed services.
- Maintains client case files, including all required reports and statistics on each client served to ensure coordinated client services to document program effectiveness.
- Performs other job-related duties as assigned.
Minimum Qualifications
- Bachelor's degree in Social Work, Psychology, Human Services or a related field.
- Three years of relevant experience.
Preferred Qualifications
- Master's degree in Social Work.
- Licensed Masters Social Worker (LMSW).
