Job Purpose

Position will provide assessment and therapy services to patients with mental health and substance use problems at community health clinics of HCHD.

Job Duties
  • Provide assessments for behavioral health problems for patients referred by health care providers at participating clinics, telehealth, and in-home service delivery.
  • Develop treatment plans for patients to address their mental health and substance use problems.
  • Provide therapeutic interventions of individual, group and family therapy for mental health and substance use problems. 
  • (Cognitive-Behavioral Treatment is the prevailing modality of such interventions, however other modalities such as IPT, PMT and Crisis Interventions may also be needed.)
  • Liaison with physicians, psychologists, nurses, and psychiatry staff regarding patient care and treatment.
  • Complete required documentation and statistical reporting in a timely manner.
  • Complete data collection procedures.
  • Provide curbside consultations to primary care providers and other clinical providers.
  • Meet with community clinicians to build relationships and discuss programming as appropriate.
  • Accommodate patients in crisis or with urgent needs.
  • Attend center staff meetings as required.
  • Attend training classes or orientations as required by the college.
  • May require travel within or near Houston. 
Minimum Qualifications
  • Master's degree in Social Work, Counseling or Psychology.
  • Experience Required: None Required.
  • Current LCSW, LPC or LMFT or completing professional licensure requirements.
Preferred Qualifications
  • Certification/Licenses/Registration Preferred: Additional LCDC.
