Summary

The  Metabolic Clinic at Baylor College of Medicine is seeking to hire a nurse with the function of navigating our patients through the clinical pathways to achieve their treatment goals in our multidisciplinary program.  Ability to provide compassionate care to the Bariatric patient; assessment skills to include the assessment of psychosocial needs; ability to plan and organize time; ability to effectively utilize resources to meet patient needs; ability to organize and pay strict attention to details; provide good customer service; demonstrates strong communication skills among team members and public speaking.  Proficient in MS Word, PowerPoint, and Excel.  Proficient in the use of Epic.

Job Duties

• Serves as a patient liaison to help patients and family members navigate the complex healthcare system as it relates to weight loss surgery. 10%
• Communicates effectively to ensure efficient patient flow. Navigates and manages the patient's pathway pre-op/post-op bariatric surgery and manages this through electronic databases and EMR systems such as Baritrax360 and Epic. 10%
• Generates periodic reports on state of the patient pipeline, as directed by the program coordinator and the Medical Director. 10%
• Works collaboratively with the Clinic Lead and Bariatric Program Coordinator. 10%
• Assists in facilitating seminars and tracking attendance. 5%
• Coordinates same day appointments to minimize visits. 5%
• Coordinates patient consult appointments as necessary to ensure efficiency and minimal travel for the patient.  Ensure patient referral appointments are made with the required specialist/testing and within the allotted timeframe to maintain the flow of pre-operative surgery clearances. 10%
• Ensures all components of pre-op clearance are completed and reviewed by surgeon. 10%
• Ensures all studies and test results are reviewed and abnormal findings addressed in a timely manner by the physician/APP.  10%
• Assists with and/or supervise tracking of clinical requirements needed to support submission of insurance approval for preauthorization for metabolic and bariatric surgery. 10%
• Follows-up on patient cancellations/no shows who do not reschedule. 2%
• Fields patient phone calls and MyChart messages and route to appropriate person. 2%
• Conducts patient outreach phone calls and post visit follow-ups. 2%
• Conducts discharge phone calls. 2%
• Provides education to the patient and family related to the specific surgical procedure, pre-operative preparations, post-operative expectations, and patient responsibilities. Provision of individualized (to include language specific) site-specific educational tools that include written, video and/or face-to-face interactions. 10%

• Participates in multidisciplinary meetings. 2%
• Reviews patient educational materials annually and make recommendations for changes or additions. 5%
• Assists Coordinator and Reviewer with development of performance metrics to measure program success. Identifies areas of improvement and develops action plans accordingly. 2%

Minimum Qualifications

  • Bachelor's degree in Nursing.
  • Four years of relevant experience.
  • Current license as a Registered Nurse (RN) by the State of Texas Board of Nursing.
     

Preferred Qualifications

  • Current BLS Certification.
  • Minimum of three years of experience in post-surgical or outpatient clinic experience; bariatric surgery related experience or Weight Loss Program related experience.
  • Certified Bariatric Nurse (CBN).
